You can access this form via the Documents page of your undergraduate application. The page appears at the end of your application and becomes available after you submit.
You cannot use a CRN from a parent or guardian’s card. The concession card must be issued in your own name to enter your CRN in your application.
If your parent or guardian holds a concession card, you can submit a copy of their card along with an application form.

Means-tested income support payments are Centrelink or other Commonwealth payments that are subject to an income and assets test (for example, Youth Allowance).
Family Tax Benefit (FTB) Part A or B is not considered a means-tested income support payment for this purpose.
If you receive FTB Part A or B and hold a Health Care or Pensioner Concession Card, you may still be eligible to apply for individual adjustments. See ‘What supporting documents are accepted?’.
Means-tested income support payments can be in your own name or in the name of your parent/legal guardian. See ‘What information will I need to supply with my application?’.
Means-tested payments from the Department of Veterans’ Affairs are also accepted.
SATAC will only use your Centrelink or Department of Veterans’ Affairs information (including your Customer Reference Number) to assess your eligibility for individual adjustments. We will not access, use, modify, disclose, or retain this information for any other purpose.
SATAC will request your data from Centrelink only if you give consent in your undergraduate application.
If you do not provide consent, you will need to submit an application form along with supporting documentation to demonstrate your eligibility.
You can find your CRN on letters sent to you by Centrelink. It is located either:
Your CRN consists of nine numbers followed by a letter, for example: 222 555 999H
Do not assume that your CRN is the same as the number you use to log in to Centrelink online services.

When you claim your Year 12 studies and enter your SACE/NTCET student number in your mySATAC application, any applicable adjustments will automatically be applied to your undergraduate application (provided you have permitted the sharing of this information with third-party institutions in your Students Online account).
